It's not really a PR stunt IMO, the logo will remain unchanged within the game and likely overseas. It's just the display requirement in the US that changed it, akin to that which required Valve to change the cover for L4D2, which remains unchanged outside of the US... well at least it did over here.

What was stopping the company from just using it's head and releasing a piece of art that they knew would go by the censors? Instead, they go for the controversial and you've got publicity when it needs to be changed.

Drowner (Read: Spitter) - Pale, bloated, glows and has transparent skin so can see the internal organs. Spits at you.

Suicider (Read: Boomer) - Cover in pulsing boils, gets close and BOOM.

Ram (Read: Tank) - Wears straighjacket and hockey mask, charges at you.

Butcher (Read: Dunno actually) - His arms have lost the hands and part of the arm flesh and his bone stumps sharp and used to slash at you.

Rather disapointed by this list. I like L4D, don't get me wrong, but this is taking it a little too far. Hopefully they shake things up a bit.

Also, what logo is what now? I missed the bit where they had a logo, and it was being censored.

Because people like creating clever logo design? :p Fucked if I know the actual answer Stu, I can only give a guess, but considering the US is the only place that seemingly has issue with it I don't believe it's a massive conspiracy theory like you do. To each their own though.

Also doesn't change the fact they're clearly not the only company to have ever had this happen to them, as I mentioned.

You're right it wouldn't be the first if it's the case. And It's not that I wouldn't think that there was the possibility if I had some reason but that more seems to be overthinking this if you really think they need a PR stunt over the logo after the trailer they launched this latest version with managed to be posted everywhere because of how awesome it is (which incidentally has that logo in it).
