What happened is he set off a bomb more or less as a diversion which killed about 7, then he went to an island hosting a Labour party youth camp in a police uniform, at which point he told people to gather around before opening fire with a handgun, shotgun and some type of rifle. Obviously due to the location he had plenty of time to rack up all those kills before police arrived.

As for why is a little more complicated, the media is portraying him as some sort of right wing nationalist nutjob which is easy to do, but the truth is he is a zionist against multiculturalism and islam as his posts prove:

The situation in Norway is currently chaotic. I still cannot fathom that this has happened. Oslo is a great city, I was just there last week visiting my friend. Also, I'm moving there next week. The blast was around 400 meters from my new work location. To think that I was walking in the blast area just the week before...
