Also have to go with the Licker crashing through the R.P.D.'s Interrogation Room window. Still to this day it kinda gives me the creeps, always grab the item, turn around, stop, hands ready on the controller to run, then straight for the door and out...think its more the audio than it is the visual, going from dead silence to that perfect combination of shattering glass with the wailing shriek of the Licker.
Another would be or was the first time through, the 3rd appearance of Mr.X in a matter of minutes. Just after 2 quick encounters at the press conference room, then there he is again, climbs over the railing of the 3rd floor balcony in the main hall, wasn't expecting that at all.
Something else terrifying, 'camera angles/positioning'...just too many to list. The big battle with Nemmy outside at the Clock Tower comes to mind though, the camera angles making you run right into his waiting loving arms.

yea the licker definitly scared the hell out of me, the crow hallway where they busted through the glass totaly got me...other moments...Lisa Trevor scares the piss out me just for the fact you cant kill her...and all you can do is run, the chain saw ganado's in re4 were pretty scary too, especially hearing that chainsaw right next to you...yikes!

The mercenaries game in RE4 held a lot of scares.
It really gets my heart rate up playing those games. I'm fine with everything besides the 'double bladed chainsaw men' on the water world level and the Garradors in the castle level.
I think it's mainly because they can get behind you and they both sound really scary and they can behead you with a single attack.
I used to be so funny with the Chainsaw men. I would just run and run and run...The whole time I could hear him going berserk, waving and revving his chainsaw around. But then I found it was much easier (and less stressful) to face them and take them out.
There's lots of scares in the RE games. I agree with many of those already posted. Fright moments, when you don't expect something and also OMG! he's BIG, he's scary and he's chasing me, moments.
