Found a Japanese to English translation of the Biohazard 3 epilogue for Ada:

A woman looks at herself in the mirror.
She used to be called Ada Wong...
But this morning she will say goodbye to that name.
It was almost time for her next mission.
    "There is no more Ada..."
There was a brand new laceration on the side of her stomach.
When the woman was Ada, it was a scar she received in order to protect the man she loved.
"This is Ada's scar, not mine."
And as she begins to cry all that was left of Ada is a weeping woman.
Compare that to the Resident Evil 3 version of Ada's Epilogue :

A woman looks at herself in the mirror.
She used to be called Ada Wong...
But this morning she will say good-bye to the name.
    "I'm not Ada Wong anymore..."
She feels her ab and thinks, "This is Ada's scar, not mine."
And as she says good-bye to Ada Wong, she can't stop her tears.
However, there isn't much time left before her next mission...
Although they have the same information, the way in which it is presented sort of lends itself to the original reason for this topic. "Why does Wesker still call her Ada?" The Biohazard 3 version of the epilogue states "There is no more Ada..." After RE3's epilogue the next place Ada was mentioned was in the second Wesker's Report where he addresses the report to Ada by name.

There it would make sense since he's probably briefing her with information about the whole Arklay facility and some brief history on Umbrella as well. As she might still be working under the guise of Ada Wong for the mission she's on, it would only make sense to call her by that.

I guess looking at it, perhaps come Resident Evil 4, she was simply assigned that code name once again?

That's really the only possible solution I can come up with...

If you were a spy, would you use your real name for an infiltration mission with the high risk of being caught and torturated 'till you lend the info they need, not just compromising your alibis but your contractors?

Nope. You use a code name. HUNK has one, it's HUNK. Ada has one, it's Ada Wong. We don't know their real names, and is up to us to figure an answer to her be calling that way 'till this days. Capcom doesn't bother too much when logic and common sense knock at the door, so I prefer the answer of PO to the matter. It's enough for me.

hey it wont happen XD capcom is too obsessed right now with making money, and it would be a while untill they make another RE game after RE5, i mean unless it makes millions....

i think theres something a bit fishy about ada, when she mentions organization s, i immediately thought oh god
