Lost in Nightmare / Desperate Escape [Spoiler Discussions go here]
I haven't gotten Lost in Nightmares yet, but I did get the costume pack.
I really like Mercenaries Reunion. It's a nice little remix of an old minigame.
i usually plant a mine on the floor just before he smashes through
This figures! I think i have a solution...or that maybe one wasn't actually needed! Next time the explosion goes off, keep an eye on the bottom right of the screen, and see if it registers an emblem.
My first attack is usually a grenade. Perhaps the blast radius from the mines or grenades is hitting the emblem.
I've only found grenades on Veteran and Pro. And never got a Machine Pistol on anything but Amateur (where as I've only got shotguns on previous runs).
The area with the symbol pieces also seems to have some odd way of spawning the enemies there, as almost no matter which way(s) you go, you will get the keepers straight in your face (they seem to only spawn near the exit under various circumstances. I guess they might use the same off-screen pop-in/spawn they used for zombies in Dead Rising.)
The enemies and weapons are for certain, which is what led me down the randomised emblems path.
Not sure how many times i've played through it now and i'm still getting surprises every so often. Keeper Of Madness beating down a door on my way out of the piano room today being a fine example!
Not sure, all i can say for definite is that they're not on amateur.
One of the weirdest things in the game i'm finding is the piano! On your first approach is the sound of someone SMASHING their hands on the keyboard, and you can hear something closing. On your second approach towards the fireplace someone is playing it, but it stops before you get to the door.
Having the Keeper Of Madness stalking the place is another reason the door opening mechanic is ill placed. Being done in game means you're defenceless for a few seconds for him to launch an attack.

Really eerie stuff.
Oh a bit Silent Hill-ish sounding, very nice. Maybe I can play it whenever PSN decides to release it.
